What is a Primary Care Network?

A Primary Care Network (PCN) is a group of General Practices that work together to provide integrated services to their local populations. This is in collaboration with other primary care staff, community groups and health and care organisations.

The aims of the PCN are to develop new ways of working together with community services, share workforce and be able to provide new services. We hope to improve the quality of care to patients in the local community with the involvement of different specialist roles. Watch this video for more information on PCNs.
